研究背景 儿童肾结石发病率以每年6-10%的增速逐年上升，成为儿童泌尿系疾病的重大问题[1-4]。尽管传统经皮肾镜取石术（percutaneous nephrolithotomy，PCNL）被推荐为＞2mm肾结石的一线治疗方案，但创伤大、高并发症多是其无法回避的挑战。 研究拟探索了内窥镜联合肾内取石术（Endoscopic combined intrarenal surgery，ECIRS），即针状经皮肾镜取石术（Micro-perc）联合逆行性肾内碎石取石术（retrograde intrarenal surgery，RIRS），是否能减少通道损伤以及提高治疗效果。本研究首次评估了Micro-ECIRS在治疗儿童复杂性肾结石中的安全性和有效性。 研究方法 本研究回顾性分析了2020年8月至2022年5月期间，13名患儿（平均年龄3.4岁，范围8个月至12岁）接受Micro-ECIRS治疗的临床结局。所有患儿均患有＞2cm的肾结石。6例患儿左侧肾结石， 5例患儿右侧肾结石，2例患者双侧肾结石，共计15个肾单位存有结石； 研究结果 手术时间：平均手术时间为70.5min（范围54-93min）。 住院时间：平均住院时间为6.4天（范围4-9天）。 血红蛋白下降：平均血红蛋白水平下降8.2 g/l（范围5.1-12.4 g/l）。 结石清除率： 术后48小时：8个肾单位完全清除结石。 术后2周：11个肾单位完全清除结石。 术后1个月：14个肾单位完全清除结石。 安全性 并发症：6名患儿出现并发症，其中4例术后发热，经抗菌素治疗后症状缓解；2例出现肾绞痛，经非甾体抗炎药治疗后疼痛减轻。无患儿需要输血或发生尿脓毒症。 Fig. 1 A, B and C: GMSV position of the patient; D: Micro-perc observed to puncture into the calyx under RIRS; E: RIRS observed under Micro-perc; F: Puncture under ultrasound; G: Schematic diagram of the operation of ECIRS composed of Micro-perc and RIRS(Micro-perc can fragment stones; RIRS can fragment stones or grab larger stone fragments); H: Stones crushed under RIRS; I: Stones crushed under Micro-perc; 结论 本研究首次证明了Micro-ECIRS在治疗儿童复杂性肾结石中的安全性和有效性。通过结合Micro-perc和RIRS的优势，Micro-ECIRS不仅提高了结石清除率，还显著减少了手术时间和住院时间。ZebraScope™ 输尿管镜在手术中的表现优异，为临床提供了可靠的工具。未来的研究将进一步验证其在更大样本量中的应用效果，为儿童肾结石的治疗提供新的解决方案。 本文仅供参考，参考文献： [1]Gregory E Tasian, et al.
